Authorization Form: This signed authorization form is required any time you wish us to fax or mail any part of your child's medical records directly to a third party like a school, daycare or camp.
Notice of Privacy Practices: This notice describes how medical information about your child may be used and disclosed, and how to access this information.
Patient Portal Enrollment Form: Print this out, sign it, and mail it to us in order to initiate a Patient Portal account. Use one form per child, and be sure to read the Terms of Service first!
